insurance company to verify your benefits and to determine if you will
need to obtain an authorization number.
We will make every effort to schedule your first appointment at a time that is convenient for you. Prior to your appointment, please print the forms located on this website. Give the completed forms to your therapist. If you are not able to print these forms for any reason, please arrive at your first appointment at least 30 minutes early in order to give yourself adequate time to complete them before you see your therapist.
